Physical Therapist (PT) - Home Health Visits
PRN/Per Diem | Poiniciana, FL and surrounding area

What You'll Do:
Provide one-on-one physical therapy services in a home health setting
Help patients improve mobility, strength, and balance
Set and manage care goals that promote recovery and independence
Evaluate the home environment for safety and mobility needs

What you bring:
Graduate of an accredited PT program
Current PT license in Florida
Valid Florida driver's license, active vehicle insurance, and clear MVR
1+ year of experience required, Home Health experience preferred
Sound judgement and a passion for patient care and outcomes
