Job Description

Position Summary:
The Social Services Assistant plays a vital role in enhancing the well-being of our residents by assisting with care planning, conducting assessments, supporting discharge processes, and acting as a liaison between families, agencies, and the care team. This position helps ensure that residents receive the emotional, social, and practical support needed for quality care and smooth transitions.
Key Responsibilities:
Attend and actively participate in interdisciplinary care conferences.
Perform ARD (Assessment Reference Date) assessments in compliance with regulatory standards.
Conduct routine departmental audits and assist in compliance tracking.
Provide case management services, ensuring residents’ needs are being met holistically.
Work closely with families, POAs (Powers of Attorney), and guardians to address concerns and communicate care plans.
Assist with discharge planning, coordination of services, and communication with external providers or agencies.
Set and coordinate appointments for care conferences and family meetings.
Support residents in submitting grievances and help mediate/resolve concerns respectfully and efficiently.
Advocate for residents' rights and needs within the social service system
Help residents connect with community resources and services, including food assistance, transportation, and mental health support
Maintain detailed and confidential documentation per state and federal regulations. Qualifications:
High school diploma or equivalent required; associate or bachelor’s degree in social work, human services, or a related field preferred.
Experience in a skilled nursing or long-term care setting preferred.
Strong interpersonal and communication skills, with the ability to work with a variety of people under sometimes sensitive circumstances.
Familiarity with MDS/ARD assessments and care conference protocols is a plus.
